Read this first: how you get paid
The truck is paid per hour only while it is actually plowing or spreading. A parked or
idling truck is not paid. Shovel labor is paid per person, per hour, only while that person
is actually shoveling. Anyone riding in or sitting in the truck is not paid. Never bill a
person and the truck for the same block of time.
- Example: three crew arrive in one truck. The truck plows 1 hour, then parks while two crew shovel 1 hour. Correct report: 1 truck hour, 2 shovel hours. Nothing else.
- Overbilling the truck while it sits, crew while they ride, or ice melt you did not use will be docked from your pay. Repeat violations can end your agreement.

3When were you on site?
4What did you actually do?
5Billable time, by the actual-use rules
Only time the truck was working. Parked, idling, or waiting time does not count. Enter 0 if no truck worked this site.
Count only time actually shoveling or hand spreading. Riding or sitting in the truck never counts. Enter 0 if nobody shoveled.
6Ice melt used at this address
Only what you spread at this address on this visit. Enter 0 if none.
7Anything else?
I certify this report is true and complete. I understand City Wide pays from this
report, that a parked truck and crew riding in the truck are not payable time, and that
inaccurate or inflated reporting will be docked from payment and can end my agreement.
